Everything governing your use of GLTrax, published by GLTrax, LLC. Documents marked signed separately are not covered by accepting the Terms — you sign each one affirmatively when you place your first lab order, and you can review or revoke them any time in Settings → Lab authorizations.

When you order laboratory testing, the clinician-patient relationship is with Wilcrest Medical Group, affiliated with Analyte Health, and they — not we — are the HIPAA Covered Entity for the resulting protected health information. Their Notice of Privacy Practices governs that relationship and is provided to you by them. We do not publish it and cannot speak for it. Ask us at admin@gltrax.com if you cannot find it.
