Everything governing your use of GLTrax, published by GLTrax, LLC. Documents marked signed separately are not covered by accepting the Terms — you sign each one affirmatively when you place your first lab order, and you can review or revoke them any time in Settings → Lab authorizations.

Documents that are not ours to publish

When you order laboratory testing, the clinician-patient relationship is with Wilcrest Medical Group, affiliated with Analyte Health, and they — not we — handle the information in that relationship under their own legal and privacy terms. Their Notice of Privacy Practices governs that relationship where applicable and is provided to you by them. We do not publish it and cannot speak for it. Ask us at legalandprivacy@gltrax.com if you cannot find it.
