FIRST. What I am consenting to

I consent to the laboratory testing I select through GLTrax, LLC (GLTrax). I understand that a licensed clinician affiliated with Wilcrest Medical Group will review my request and may sign a laboratory requisition if clinically appropriate. I will provide a specimen at a patient service center, usually by venipuncture (a blood draw from a vein), and an independent laboratory will perform the testing. The clinician may decline to order a test.

SECOND. Collection risks

Blood collection is common and generally low risk, but may cause pain or discomfort, bruising, bleeding, hematoma, dizziness or fainting, and, rarely, infection or nerve irritation. More than one vial may be drawn. A specimen may be insufficient, damaged, or unsuitable for testing, and repeat collection or testing may be necessary.

If I have a bleeding or clotting condition, take a blood thinner, have fainted during blood draws, or have concerns about a blood draw, I will consult my own healthcare provider before testing.

THIRD. Limits of testing and results

I understand that laboratory results:

• are a snapshot in time and are not, by themselves, a diagnosis or a complete assessment of my health;
• may be affected by fasting, hydration, time of day, recent illness or exercise, medications, supplements, specimen issues, or laboratory error;
• may be falsely positive or falsely negative, require repeat or confirmatory testing, or be abnormal, unexpected, or distressing;
• use reference ranges that are population-based and may not reflect my individual circumstances, including pregnancy or chronic conditions; and
• do not test for every condition; a result within a reference range does not rule out disease, and a result outside a reference range does not establish disease.

I understand that receiving results may cause anxiety or reveal information I did not expect to learn. I am responsible for reviewing my results with my own qualified healthcare provider and obtaining any needed follow-up care.

FOURTH. GLTrax is not my clinician

GLTrax is a wellness platform. The clinician who reviews my testing request is not my primary or treating provider and will not provide ongoing care through GLTrax.

My relationship with Analyte Health, Wilcrest Medical Group, the ordering clinician, and the performing laboratory or collection site is separate from my relationship with GLTrax and is governed by those parties’ respective terms, notices, policies, and clinical practices. GLTrax does not control, endorse, adopt, incorporate by reference, or accept responsibility for any third-party terms, notices, policies, clinical practices, websites, or their availability. Any change to a third party’s terms, notices, or policies does not amend this consent, the GLTrax Terms and Conditions, or the Lab Services Terms.

Ranges, flags, trends, summaries, and AI-generated content displayed in GLTrax are informational software outputs, not clinical findings or medical advice. AI-generated information may be inaccurate or incomplete. I will not start, stop, change, or substitute any medication, including a GLP-1 medication, based on a laboratory result or GLTrax content without consulting a licensed prescriber.

FIFTH. Electronic delivery

I consent to receiving result-availability notices and related communications electronically through the GLTrax app and by email. GLTrax will not send laboratory result values by email; email notices will direct me to sign in to my account. Electronic communications can be delayed, intercepted, misdirected, or accessed without authorization. I will keep my contact information current, safeguard my account credentials, and not share my account.

SIXTH. Alternatives and cost

I do not have to obtain testing through GLTrax. I may instead seek testing through my own healthcare provider, who may know my history, interpret results with me, and arrange testing that may be covered by insurance or cost less. I freely choose GLTrax testing with this understanding. I understand that testing ordered through GLTrax is self-pay, as further described in the Lab Services Terms.

SEVENTH. My consent and withdrawal

By signing, I represent and agree that:

• I am at least 18 years old, am acting for myself, and am consenting to testing on myself only;
• I have read and understand this consent, including the collection risks and testing limitations;
• I have had the opportunity to ask questions before ordering by contacting support@gltrax.com;
• I am consenting freely and voluntarily; and
• I may withdraw this consent by cancelling my order before my specimen is collected. Cancelling an order is a separate act from revoking an authorization: cancelling stops that order, while revoking a Lab Order Authorization or Records Release Authorization is done in Settings → Lab authorizations, applies to future orders and disclosures, and does not by itself cancel or refund an order already placed.
• Withdrawal takes effect when I cancel. It does not stop testing already underway, does not undo a disclosure already made, and does not reverse any other action already taken in reliance on this consent while it was in force.

NINTH. Copy and record

I am entitled to a copy of this consent. GLTrax will make the signed and dated copy, including its document version, available in Settings → Lab authorizations.

Document version 2026-09-22. This consent is signed as part of placing a laboratory order. The version presented at signature and the date and time of signature will be retained with the signed copy. This is the version published today, which is not necessarily the version that governs an order already placed.
