1. What I am consenting to

I consent to the laboratory testing I have selected through GLTrax. I understand that a licensed clinician at Wilcrest Medical Group, affiliated with Analyte Health, will review my request and, if clinically appropriate, sign a requisition ordering the tests; that I will attend a patient service centre where a specimen — typically blood drawn from a vein — will be collected; and that an independent laboratory will perform the testing.

2. Why someone might want these tests

The panels offered through GLTrax measure markers commonly followed by people taking GLP-1 medications, including blood sugar control, insulin, cholesterol and lipids, kidney and liver function, and — in the larger panel — thyroid, inflammation, pancreatic, cardiovascular risk, and nutrient markers. Tracking these over time can give me and my healthcare provider information about how my body is responding.

3. Risks and limitations I understand

Risks of the blood draw. My specimen is collected by venipuncture — a needle into a vein in my arm. It is a common, low-risk procedure. It can cause discomfort or pain, bruising, a hematoma (a firm lump of collected blood under the skin), bleeding, light-headedness or fainting, and — uncommonly — infection at the puncture site, or nerve irritation. The collection site's staff, not GLTrax, perform the draw.

More than one vial may be drawn, and occasionally a second visit is needed — for example if a specimen clots, is insufficient, or is damaged in transit. If I have a bleeding or clotting condition, take an anticoagulant or blood thinner, have fainted during a blood draw before, or have a fear of needles or blood, I should speak with my own clinician before ordering.

Limitations of the results. I understand that:

• A laboratory test is a snapshot in time, not a diagnosis, and no single test can tell me whether I am healthy.
• Results can be affected by fasting status, hydration, time of day, recent illness or exercise, medications, and supplements.
• Laboratory error, specimen problems, and false positive and false negative results are possible. A test may need to be repeated.
• Reference ranges are population statistics. A result inside a range does not mean nothing is wrong, and a result outside one does not mean something is.
• These panels do not screen for every condition. A normal result does not rule out a disease the panel does not test for.
• A result may be abnormal, unexpected, or distressing, and may lead to further testing, cost, or anxiety.
• Reference ranges differ during pregnancy and with certain chronic conditions, and a result read against a general adult range may be misleading in those circumstances.

No one is monitoring my results for me. I understand that no clinician at GLTrax reviews my results, that the clinician who signed my requisition is not my treating provider and will not follow up with me, and that it is my responsibility to review my results and take them to my own healthcare provider.

What it means to receive results. A result may tell me something I was not expecting and did not want to know, and it may be distressing. Once I have read a result I cannot un-know it. If I am unsure whether I want this information, I should speak with my own clinician before ordering rather than after.

If I share my results with my own healthcare provider, they may become part of my medical record. Information in my medical record can be seen by others who have lawful access to it, including insurers, and may be considered in underwriting decisions for products such as life or disability insurance. This is a consequence of sharing results with a provider, not of ordering them through GLTrax.

4. GLTrax does not interpret my results

I will not start, stop, or change any medication — including a GLP-1 — based on a result I see in GLTrax. Those decisions belong to a licensed prescriber who knows my history.

I understand that GLTrax uses artificial intelligence to generate insights from my data, that AI output can be inaccurate or incomplete, and that I should verify anything health-related with a qualified professional.

5. Electronic delivery and its risks

I consent to receiving my results and related communications electronically, through the GLTrax app and by email notification. I understand that email is not a secure form of communication, and I accept the risk of loss, delay, interception, or unauthorized access or disclosure inherent in electronic communication. I understand that in rare cases data security protocols can fail, causing a breach of privacy of my health information.

I will keep my email address current and my account password secure, and I will not share my account with anyone. I understand GLTrax will not send result values by email — only a notification to sign in.

6. Review, and being contacted

I agree to review my results within 14 days of being notified they are ready. If I do not, I consent to GLTrax contacting me by other means — including telephone, text message, or certified mail to the contact details on my order — to confirm I received them.

7. My alternatives

I understand I do not have to use GLTrax to obtain this testing. I may instead see my own healthcare provider in person and have testing ordered there — which may be covered by my insurance, may cost me less, and comes with a clinician who will interpret the results and follow up with me. I am choosing to order through GLTrax freely, understanding these alternatives.

8. Cost

I understand this testing is cash-pay only; that I pay in advance and out of pocket; that no claim will be submitted to insurance, Medicare, Medicaid, TriCare, or any other federal or state healthcare programme; and that the amount is unlikely to count toward any deductible. I understand that the amount shown to me at checkout is the full amount for my order and that GLTrax will not charge my payment method again afterwards, as described in the Lab Services Terms.

9. My consent

• I am at least 18 years old and am consenting on my own behalf, for testing on myself only.
• I have read and understand this consent, including the collection risks, the limitations of results, and what it means to receive them, all in Section 3.
• I have had the opportunity to ask questions, and I know I can ask more at admin@gltrax.com before ordering.
• I am consenting freely and voluntarily, and I may withdraw my consent by cancelling my order before my specimen is collected.
• I am entitled to a copy, and a signed, dated copy is available to me at any time in Settings → Lab authorizations.
