Lab Services Terms

1. Who Does What
Summary: Four parties: you, us, the clinician who orders the test, and the lab that runs it. We are the software in the middle — we never touch a specimen or read a result.

None of the clinicians, laboratories, or collection sites is our employee, agent, or subcontractor, and we do not control them. We are not responsible for their acts or omissions, including the quality of a draw, the accuracy of a result, or the conduct of staff at a collection site.
2. Eligibility
You must be at least 18 years old to order laboratory testing. We verify this against the date of birth on your order and will reject an order that does not meet it. You may order testing only for yourself. Ordering on behalf of another person — including a spouse, parent, or child — is not permitted, and an order placed with someone else's information may be cancelled without refund.
Laboratory testing is available only where permitted by law. Testing availability depends on your state. Some states restrict, condition, or prohibit consumer-initiated laboratory testing, and some tests are unavailable in some states. Availability varies by state and may change at any time, including because a state changes its law or because our laboratory partner changes where it operates. We may decline or cancel an order on that basis, before or after you pay. Where we do, you receive a full refund of the amount paid, including any cancellation fee, on the same terms as any other order we cancel before testing (Section 13).
3. What You Sign Before Ordering
Summary: Three separate signatures, one time. Accepting the general Terms is not enough — these are distinct legal documents.
Before your first order, and again whenever we publish a new version or you revoke a prior signature, you must separately and affirmatively sign:
- Lab Order Authorization — authorizes us to submit the order on your behalf and to disclose your information to Analyte Health, Wilcrest Medical Group, and the performing laboratory.
- Records Release Authorization — authorizes Analyte Health and Wilcrest Medical Group to release your results and related health information back to us. Without this we cannot receive or show you your results.
- Informed Consent for Laboratory Testing — your consent to the testing itself, and your acknowledgment of its limits and risks.
Each is signed separately, timestamped, versioned, and retained. You can view what you signed and when, and revoke any of them, in Settings → Lab authorizations.
4. Panels and Pricing

What you pay has two parts. The laboratory and clinician fees are passed through to the performing laboratory and the medical group at the amount they charge us, with no markup — we retain none of that component. The technology service fee is what we charge for processing, organizing, and handling your lab request. It differs by panel: a larger panel takes somewhat more coordinating, so the fee is higher on the Plus Panel than on the Core Panel. It is not a percentage of the laboratory component and rises far more slowly than it does.
Itemization of all fees is available on request. The prices above are totals. If you want the amount of each component for an order, you can request it by emailing admin@gltrax.com, and we will send the laboratory and clinician fees, the technology and service fee, and the total as they were recorded when you paid.
Every panel purchase includes three months of the tracking plan free, on every order rather than only your first. If you already subscribe, the three months are added to your subscription; if you do not, a subscription starts free for three months and no card is required for it. Buying another panel while free months are running adds three more. When they end without a payment method on file the subscription simply ends, and you are never charged for it. Full terms are in Section 8.1.2 of the Terms and Conditions.
A panel is a one-time purchase. It is not a subscription, it does not auto-renew, and it is not included in or discounted by any subscription. A GLTrax subscription covers the tracking product only — the daily check-in, trends, summaries, reports, and export — and neither grants nor gates the ability to order testing. You may order a panel whenever you want one, with or without a subscription.
Prices are per order, in U.S. dollars, and exclude any applicable tax. The available test menu, laboratory options, and pricing may be added to, removed, or changed at any time, including because our laboratory partner changes what it offers or what it charges us. We do not guarantee that any particular panel or test will remain available. The price shown to you at checkout is the price for that order; a later change does not affect an order already paid for.
A panel is a fixed set of tests. You cannot add or remove individual tests from a panel through the Platform.
5. Placing an Order
You are responsible for the accuracy and completeness of the information on your order — your legal name, date of birth, sex, phone number, and the email address results should go to. We submit what you give us. Inaccurate information can delay your order, cause it to be rejected, make a specimen impossible to match to you, or produce a result that is wrong or unusable, and an order that fails for this reason may be non-refundable.
6. Payment and Cash-Pay Only
Summary: You pay us up front, out of pocket. No insurance, no Medicare, no Medicaid.
Laboratory orders are paid in advance through our payment processor, Stripe, Inc. Payment is due before the requisition is issued; an unpaid order is not submitted.
Cash-pay only. These services are provided on a self-pay, out-of-pocket basis. We do not bill health insurance, and we do not provide superbills, CPT-coded receipts, or documentation for insurance reimbursement. Neither you nor we will submit a claim for these services to Medicare, Medicaid, TriCare, or any other federal or state healthcare programme, and we are not enrolled with any of them. Amounts you pay are unlikely to count toward any deductible.
If you have insurance that would cover this testing, ordering it through your own healthcare provider may cost you less. You are free to do that instead.
7. No Additional Charges After You Order
Summary: The price you pay is the whole price. We do not bill you again after your order.
The amount shown to you at checkout is the full amount for that order. We do not charge your payment method again after you place an order. There are no reflex fees, no pass-through laboratory surcharges, and no add-on charges from the collection site billed to you by GLTrax.
If the performing laboratory runs a confirmatory or follow-up test on your specimen under its own protocols, that is a clinical decision between the laboratory and the ordering clinician. It does not produce a further charge to you from us.
We do not mark up laboratory pricing. Laboratory and clinician fees in Section 4 are passed through at the amount our laboratory partner and its affiliated clinicians charge us; what we retain is the separate technology and service fee described there.
8. Specimen Collection
After your order is accepted and paid, a requisition is issued and you attend a patient service centre to give your specimen. You must bring government-issued photo identification matching the name on your order. Some tests require fasting or other preparation; where they do, we will tell you before you go, and failing to follow the preparation may make the result invalid and require a repeat test at your expense.
The collection site and its staff are third parties. Any injury or personal damage occurring at a collection site is a matter between you and that site, and you agree to indemnify us in respect of it as set out in Section 17 of the Terms.
9. Requisition Validity
A requisition is valid for 90 days from the date it is issued. If you do not attend the collection site within that period, the order expires and is treated as cancelled. An expired order is not automatically refunded; contact us at admin@gltrax.com and we will, at our discretion, either reissue the requisition or convert the amount paid to credit toward a future order. We may reissue a requisition once at no charge.
10. Results Delivery
Results are typically available 2–5 business days after collection, and some tests take longer. This is an estimate, not a guarantee. Turnaround depends on the performing laboratory, the specific test, specimen quality, and whether reflex testing is triggered, none of which we control.
Results are delivered to you in the Platform, and we notify you at the email address on your order that a result is ready. Email is not a secure form of communication. We do not send result values by email, only a notification to sign in. You are responsible for keeping your email address current and your account secure, and you accept the risk of loss, interception, or unauthorized access inherent in electronic communication.
We do not guarantee uninterrupted access to results. Access may be interrupted where our laboratory partner suspends or revokes our access to its portal, during maintenance or an outage, or where your account is suspended. If delivery is interrupted, we will tell you and work to restore it; if we cannot deliver a result you paid for, you may request the result directly from the performing laboratory or the ordering clinician, who hold it as your medical record.
11. Your Duty to Review Results
If you have not viewed a result within 14 days of notification, we reserve the right to contact you by other means, including telephone, text message, or certified mail sent to the contact information on your order, to make sure you have received it. You consent to being contacted this way for this purpose. This does not create any obligation on us to monitor, chase, or interpret your results, and it does not make us responsible for a result you do not read.
12. We Do Not Interpret Results
Reference ranges, flags, trend lines, colours, and any AI-generated commentary shown alongside a result are contextual information generated by software. They are not a clinical finding, a diagnosis, or an opinion that anything is or is not wrong with you. A result inside a reference range does not mean you are healthy, and a result outside one does not mean you are ill — ranges are population statistics, and interpreting a value requires knowing your history and context.
A laboratory test is a snapshot. Results can be affected by fasting status, hydration, time of day, recent illness, medications and supplements, and laboratory error. Do not start, stop, or change any medication — including a GLP-1 — based on a result you see here. That decision belongs to a licensed prescriber.
Our laboratory partner is not responsible for our interpretation, use, or communication of results after delivery, and neither we nor it is responsible for what you decide to do with them.
13. Cancellation and Refunds
Summary: Free cancellation for one hour. After that, we can usually still cancel before your specimen is collected, minus a $35 fee. Once the lab has run the test, there is no refund.

Refunds are issued to the original payment method and typically appear within 5–10 business days depending on your bank. Where an order expires unused, Section 9 applies instead of this Section.
14. If You Revoke an Authorization
You may revoke your Lab Order Authorization or Records Release Authorization at any time in Settings → Lab authorizations. Revocation takes effect when we receive it and applies going forward. It does not undo anything already done in reliance on the authorization while it was in force.
Practically, revoking means:
- We stop receiving results for you. If you revoke after a specimen has been collected, we may be unable to show you that result — you would need to obtain it directly from the performing laboratory or ordering clinician.
- You cannot place a new order until you sign a fresh authorization. You can do that at any time.
- It is not a cancellation. A pending order stays pending and is refunded, or not, under Section 13.
- Results we already hold remain in your account until you delete them. Delete them separately if that is what you want.
15. Pregnancy and Special Circumstances
Reference ranges for many of the tests in our panels — including glucose, A1c, thyroid, and lipid measures — differ during pregnancy, and a result interpreted against a general adult range can be misleading. If you are pregnant, may be pregnant, or are breastfeeding, discuss testing with your obstetric or primary care provider before ordering, and interpret any result only with them. The same caution applies if you have a chronic kidney, liver, thyroid, or haematologic condition, or take a medication or supplement known to affect these values.
16. General
These Lab Services Terms are part of the GLTrax Terms and Conditions, and the general provisions there — including the disclaimers of warranty, the limitation of liability, the indemnity, governing law, and the arbitration agreement and class action waiver — apply to laboratory services in full. Where these Lab Services Terms and the general Terms differ on a matter specific to laboratory testing, these Lab Services Terms control.
The authorizations and informed consent you sign are separate agreements and are not superseded by these Terms. If an authorization is narrower than these Terms as to how your information may be used, the authorization controls.
